AO: This is a digitized copy of the signature page of the informed consent that was reviewed by Participant BCMWAS11 prior to interview # 191127_001 and signed after the interview. While the participant stated their willingness to be named and wrote out their full name in the signed consent form, given that I interviewed others within the same research organization who wished to be anonymous, I made the determination to redact the name since naming the individual would mean that the research organization could quickly be identified and those who wished to be anonymous within the organization potentially could also be more easily identified.
